Physician Referral

 

You will need a referral from your physician to have your insurance pay for your physical therapy treatment.

 

Referrals are accepted from persons licensed to practice medicine: nurse practitioners, surgeons, dentists, podiatrists or chiropractors.

 

 

Take this referral form to your medical provider, then have it completed and faxed or delivered to Physical Therapy of Lake Charles. This is provided as a convenience to your referring physician and serves as a written prescription that most insurance companies require for payment.

 

Physical Therapy of Lake Charles will accept an initial referral by fax, by phone or by email from your physician, and we will obtain the needed signed prescription within days of your referral; or you may bring the signed referral form with you to our office.

 

We will also accept the referral forms from other clinics or hospitals.

 

REMEMBER: You, the patient, may choose where you receive your physical therapy.
